Your medical records can be sent to your new provider or to you. You must sign a release form before your records can be released, even to yourself. Most medical records are sent by fax. This is a free and secure way to send your records. You are strongly encouraged to have your records faxed to your provider.
A hard copy of your records can be mailed to you. There is a $30 fee to print and mail your records to offset the cost of producing the records. You can pay by credit card or by check.

If you pay by check please allow 14 business days to receive records.
Lastly, you can request that your records be sent to you by email. Email is not considered secure and HIPAA compliant. If you want your records sent by email, you must sign on the release form that you understand that email is not secure.
Steps to request a copy of your medical record:

- Complete and sign the form and select where and how the records are to be sent. After completing the form, sent it back to us as follows:
